Primary Care @ Home doctors are trained in remote care and house call medicine. They are Board Certified and experienced at delivering primary care. Primary Care @ Home physicians are licensed to practice medicine and have the training to understand your health history, provide a diagnosis and prescribe treatment.
Primary Care @ Home can provide physician house call medical care to multiple residents (usually on the same day) for patients in group residential settings such as assisted living facilities and senior living communities.
Through this service we can also serve as the medical director of your facility, providing the care your residents need at a lower cost to the facility.
Scheduling appointments and taking residents to a physician’s office can be a stressful experience for patients and a coordination nightmare for caregivers. We visit facilities and care homes to provide your residents with medical care in familiar surroundings.
We visit as many or as few of your residents as would like to sign up with our services.
Our program of care works best when our physician can visit on regularly scheduled days, seeing multiple residents in one site visit per day.
